Add Additional Client ID to Form

I would like to add a button that says "+ Client ID" to this form so that users can add multiple Client IDs on a single form. Any guidance would be appreciated. Thank you!

What is client ID? A text or a lookup?

If it is a lookup then you need to create relationship N:N and add a grid of client ID on this form which will then allow you to add multiple client ID.

If it is text then you need to create a new entity called clientID and then create relationship as describe above.
