Missing Form in account entity after published

Hi guys, 

I created a solution and add a "new form" in side the account entity. 

After I published it. I cannot find the option at the front page.

As the following image, the left image is my customized page shows I already created the form.

The middle page is mine page, with no option for the new account's form. 

And the right page is the one that the tutorial shows(a account list for you to choose which form you need)

Did anyone face the same problem as mine?

    Have you added the form to the Model-Driven app? Don't forget to save and publish. Also make sure the security role(s) for the form are correct.
