Exception Message: ISV code reduced the open transaction count.

Hello Experts,
 I am new to CRM and currently facing an issue in the plugin.
The plugin is creating entity records using service. Create(entityname). Now for some condition this is working but for rest it gives error :
 
Exception Message: ISV code reduced the open transaction count. Custom plug-ins should not catch exceptions from OrganizationService calls and continue processing.
 
Please suggest an approach to resolve it.
