Issue with rendering notes on my portal

Greetings all,

I am having issues while trying to render notes in my portal area.

Here are some details -
* Working with customer service hub
 
* This is the  portal form am using, and have enabled all permissions in it.
 
 
*  On customer service hub model driven app already added a note and saved all settings.
 
 
* Provided table permissions on portal management as well
