I am taking on support for a clients CRM and need to start with updates to fields, adding removing etc but ballooning into general customisation as time goes on.
What is the correct way to make these updates in a controlled manner? I can make the changes manually in sandbox, get sign off and then manually recreate in production but this seems to duplicate effort. I know some add-ons use managed solutions but is this an appropriate use case?

If it is , you could create one new managed or unmangerd soltion in you Sandbox instance and add related customizaed entity with your required customization as your solution components. Then you could export the solution and re-improt in your Product Instance which needs no more duplicate effort.
