Get Agent Script Answer Id and Text in data parameters - Unified Service Desk

I have a scenario where the client wants us to make an entry into a custom entity whenever an Agent Script Task or Answer is clicked.  
I am able to get the agent script task name from data parameters when a task is clicked but for agent script answers i am unable to get the answer text and id into the data parameter of USD when i click the answer.

I don't want to create an action on each agent script answer as this would be cumbersome in case of too many agent script answers.

Is there any way i can achieve this ?
