How to add a placeholder or Watermark in model driven app. like the image i add here , i want in model driven app.is that possible, if yes, then please guide me...

i have tried using JavaScript , but I can't achive it,

link which you given me also checked and the portal is not working ,

And is there any code that you can give me to set watermark / place holder for my model driven app if there any idea then please guide me .
