How we can add the link into the Appointment description box
I have to below link into a description box same as showing
Any help would be great.

I believe you need to use Rich Text Control which supports Hyperlinks:

I have to set only when form type =1 i have to show the link

I have to add a link URL to the Description box when an appointment is created on the form onload . when anyone clicks on that link it will redirect to that page
