Survey Template to gauge skill level and interest level in our CRM

Hello!

I am assisting our team as the new CRM Coordinator and I'm wondering if anyone has a survey template that they've shared out with their users to obtain interest level, skill level, pain points, and what other training they would they like to see? something of that nature.
