When i open the Users screen, i cannot find the user SysHealthServiceUser..
Is there something i need to enable?

I believe this issue has to do with the USER not existing in the system.. (not the access).
Any ideas?
If you are using custom security role , make sure to have proper access give to that role to the Purchase order approval menu item. In your case i think its going to be PurchTableApprovalApprove. Also take a look at below article and see if it helps.

No it wasn't deleted.. not sure where/how it went ?
Perhaps you have deleted it? The system service accounts are documented here: docs.microsoft.com/.../pre-configured-system-accounts
