Unable to create Menu in left side

Hello Experts,

I am new to power portal customizations. I have a requirement that I have to design a main form exact like below screen shot.

Kindly suggest me and do let know the steps to design the form.

Mainly I have to add these menu items in left side and when we will click on any menu then the form should be open in right side. Kindly suggest regarding the same.
