How to enable new CI-J Contact Insights form on custom contact form?

I am working on transitioning a customer from Outbound to Realtime. They have the Contact Insight from Outbound visibile on a custom Contact Main form. As there is a new one for CI-J, I want to enable this one too in the form. Is this a control I can add to the form? If so, what is the name of it? I want them side by side as in the example shown here:

    A custom control is available "Insights Control" displaying the expected result.

    Are you sure you aren't referring to the control for the old Outbound Marketing Data. I found one which seems to display both the old data and the new. It is  called TabbableInsightsControl.

    TabbableInsightsControl is indeed the right control for the new CI-J
