Modified By Field Value

I have v9 dynamics CRM. In that for any task creation, Modified By value is showing as /System/ instead of created user. While same is not in the case of V8 version.
I have checked for every possible custom code and processes/Plugins calling user context but nothing impact that behaviour.
Could anyone help me in knowing the reason or any resolution of the same.
