GL Detail Inquiry screen does not appear in order to print.

In the last couple of days, when attempting to print within GP, the GL Detail Inquiry screen does not appear.  No error, nothing.  The screen just does not open.
 
I've rebuilt dictionaries, reports and repaired Dynamics GP.  Still, the screen doesn't appear when clicking the print button.
 
This is for all users.  Made sure there was no noprint strand in dex.ini
 
Any insight on this issue?  We had a Table Hierarchy Inconsistency error prior to the print screen issue, that has disappeared along with the missing pop up.
