One common cause is when you have the same format for two different number sequences e.g. AR and AP journals and both have this same format and starting number: ABC-000001.
Ax writes a record into the SUBLEDGERVOUCHERGENERALJOURNALENTRY table upon posting.
A check is done to see whether the voucher exists, and when it does, then we can get an error because even though the voucher number is associated to a different reference, via a different number sequence code, you cannot post because it already exists, because Ax only validates the voucher number and not the voucher number/reference combination.
It is always best to use a unique format for transactional related number sequences.
