Custom Labels on a form

Hi All, I feel stupid having to ask this but.. is there a way to put a simple label inside a form?

I'm working on a simple form where I need to put few text labels and additional 2 options (optional) for the users to select. Something like this:
 
 
LABEL, CUSTOM TEXT                                                           PERCENTAGE COMPLETED                                               NOTES
Step 1                                                                                     Choice - 25/50/75/100%                                                Optional
Step 2                                                                                     Choice - 25/50/75/100%                                                Optional
Step 3                                                                                     Choice - 25/50/75/100%                                                Optional
Step 4                                                                                     Choice - 25/50/75/100%                                                Optional
 
and so on, around 12 steps. Note, not every step will require percentage column, but all will have notes next to it.
I thought it be a straight forward task, add a label, choice where required and a notes section but it seems there is no option to add custom text labels into a form.
Any help appreciated

    Our team analyzed your needs and came up with a possible solution, here are some simple steps to help you understand.
    Step1, write the HTML page (the code is too long just paste the highlighted part)


    Step2, Add the HTML page to the solution as a Web Resource.
    Step3, Add Web Resource to Form

    You can achieve this with custom pages, PCF control or HTML.
