How we can display Medical card details in benefit to employee after employee enrolled and service provider dispatched medical card.

Following up on this one - are you looking for something to display/attach the actual medical provider card visibly within the user interface? In most forms, you can add an attachment to a 'document' so you might be able to do that here as well, but if you are thinking of something where in Employee Self Service they can download their provider card directly, there is not really a straightforward way to do so. Is this more of what you were indicating?
