We have a user who is getting the 'User ID xxxxx is already logged into the company xxxxx. Would you like to view the current logins?' message. After hitting yes, the Current User Login Activity table is showing with no entries. User is unable to login to the account. There are no entries in the Dynamics..Activity table for the user.
Check the DSN on the workstation and make sure it is pointing to the correct server; Is this a single or multi-tenant GP installation?
Workstation is pointed to the correct server. It is a single tenant GP installation.
If you look in the actual ACTIVITY system table in SQL, do you see a record for that user?
If so, delete it and then the user should be able to login to Dynamics GP.

There is no record for the user in the ACTIVITY sql table.
Do you have any third-party products installed? Have tried stopping/start SQL on the server or rebooting the GP server?
This sounds weird...
As Richard suggested, I suspect you may have some add-on installed to GP, like this would be a behavior of GP PowerTools when you enable the ACTIVITY clean-up option... It checks for stranded user entries in the ACTIVITY table every time a new user logs into GP or switches companies...
Ultimately if that's not the case, you could start a SQL trace from the back-end to check what's going on and what would trigger the message.
We have an app, AvaTax, installed.
SQL server has been rebooted a few times since this started. Issue persists.

First of all, you should not have to reboot your SQL server all the time... a SQL server should be running smoothly for months on the go when it's properly configured and not need regular reboots, except for system updates once in a while.
I don't think AvaTax is your problem, but I have never used that, so can't be 100% sure about it.
One way to TEST if the add-on is the culprit is to install GP bare-bone without any ISV on a new workstation (or rename the existing GP folder and run a repair on your GP client, which is going to recreate GP without the ISV's).
The next thing would be to perform a DEXSQL trace when starting GP to see if anything goes on during that process.

In addition to Beats suggestion, does this only happen for this one user? Does it follow the user from workstation to workstation? Does this user have anything in its startup group that may be causing the dual login issue? Can you paste a copy of the DYNAMICS.SET file into this case.
There ended up being corruption in the database. We had to restore to a previous point in time and the user was able to log in again.
