For this case, kindly do the inspect element of the button id and check the command associated with it.
Once you have the command , you can check associated display and enable rules.
Just remove it once and publish the solution in ribbon workbench and check if button is visible or not.
If button is visible then need to check the enable and display rules in it.
Also if it is an online instance , you can use command checker feasibility as explained below :-
These is a new command available inside the ribbon to help us troubleshooting the ribbon issues.
This can be useful when you are investigating a button that is not showing on the UI, you can search the button, review the related rules and see how they are evaluated.
This works only for UCI.
How : Add to your UCI url the following “&flags=FCB.CommandChecker=true&ribbondebug=true”
Example: https://orgf27784e8.crm.dynamics.com/main.aspx?appid=16063a8f-6dc9-e911-a980-000d3a3b535a&pagetype=entitylist&etn=account&flags=FCB.CommandChecker=true&ribbondebug=true
Result:



This will tell you which enable or display rule computes to be false and thus your button is not visible in UCI.
