Deployment Issue with Managed Solution Error

I have a managed solution that I have deployed several times successfully to our environments (Stage, UAT, Production) in the past, but recently we had added some SLA KPIs for upcoming SLA enhancements. Since this happened our solution will deploy to our lower environments (STAGE, UAT), but we are getting an error when we try to deploy to our Production environment.

Error: Import failed: Cannot insert duplicate key row in object 'MetadataSchema.EntityRelationship' with unique index 'ndx_for_entitykey_UniqueEntityRelationship'. The duplicate key value is (0, Jan 1 1900 12:00AM, mss_purchasingtask_SLAKPIInstances).

I have looked for any KPI instances in Production, but there are none in the table. It seems like a metadata issue, but I cannot find it.

Anybody have this type of issue before or have a troubleshooting path I can take?
