REAL TIME Marketing - Creating a task for operations team when a form is submitted

I have a question about creating a task or workflow when a form submitted. 
 
App feature: Realtime Marketing Forms
Form field: I have used a textarea input which maps to a contact /Description/ field. The idea is to collect questions from customers. This input ideally will then be added to an /Activity task/ and assigned to our operations team so that they can respond to the customer. 
Teams involved: Marketing (access to both CS Hub, and Marketing App), Operations team (access to just CS Hub)
 
My question is - how can I automate this form workflow so that any new form submission that comes through, will then be posted as an activity for the operations team on Customer Service Hub, to see and respond to the customer question?
 
At the moment, I have set up an automated journey that sends out an informational email to customers who submits an enquiry, but I'd like this to be more personalised by also flagging the form response with our operations team so they can personally follow up.

    Hi Partner,
    You mentioned that the text area input mapped to the 'description' field of contact.
    So the 'description' field will be updated when you submit the form.
    You can use changes to the Description field as a trigger to create a workflow, use that workflow to create an activity task, and assign the task to the team you need.
    --Settings > Process
    --Trigger
    --Steps:
    --Set Properties:
    Get value from contact to populate fields on the Task
    Set owner with one team you want.
