dynamics 365 survey link not working

we have made a survey on dynamics 365 but the link is not working, it just load for 20 seconds and then throws up an error 500 and we just cannot access it.

    How did you generate the link? Perhaps a screenshot or .gif of this not working could be helpful to troubleshoot.
