How to display custom form?

Hi,

I've created different forms for different objects such as lead etc... it works.

But I've created a custom form for account and it's not displaying. I don't have any drop down option to select between my forms. It displaying only one form.

I've already have affected the form to my security role but it's nor working.

Could you help me please?

You need to allow the target page in the app designer.
