Mandatory Questionnaire on requisition for procurement category?

I have set up a test questionnaire with one question and added it to a procurement category. I can still submit a requisition for this category though without seeing the questionnaire. I have ticked the 'active' box, do I need to set some other parameter?

Also ideally I would like to make one of the questions -what is the 'x' dimension ? and have a drop down box linked to a new dimension I have as the answer. I need to force the new dimension onto the requisition for certain procurement categories, but depending on the project posting rules, the transaction can be posted to GL accounts where I can't set the accounting structures to be mandatory for the new dimension, so trying to use this approach instead.

Can the answers given be used to populate any fields, or are they just recorded as the 'answer'?
