User randomly getting CC'd on outgoing emails

We are on 2016 v8.2 on-prem and have a user who is getting her FROM email address added to the CC line on emails that the user sends. The user is noticing this after the email has been sent and is not adding the CC line. We are using server-side sync for Exchange.
