Collect small survey on existing form

Hello,

We have an entity A, where we have an entity B as subgrid. In this subgrid we have a record for every year business plan. In this form, we have 4 multiple lines of text fields to collect each quarter plan( that would be 4 quarters). Lately, we have a requirement to have a 6 question survey in this Entity B form so that users can do business performance of clients during pandemic. The 6 question survey also needs to be collected every quarter.

For example :

In B entity form we have 4 mulitple lines of text fields.

Q1
Q2
Q3
Q4

They want to have 6 question survey for each quarter in that entity B form.

What would be the best way to approach this? We are using Dynamics 365 8.2 on premise.
