Dynamics CRM App for Outlook Manifest File Location

We are having issues pushing the Outlook App to our clients in Dynamics 365. The error is:

This app can't be installed. The manifest file doesn't conform to the schema definition. The 'schemas.microsoft.com/.../1.1:AppDomain' element is invalid - The value '' is invalid according to its datatype 'schemas.microsoft.com/.../1.1:LongString' - The actual length is less than the MinLength value

We suspect that the manifest file is missing the element AppDomain value. So my question is "Where does Dynamics 365 get the manifest file for App For Outlook"? 

We want to find where this manifest file is located or where does CRM get the manifest file so we can find out where the issue is. Thanks in advance!

    Hi I found out that the issue was during the Claims Based setup, if you choose a ADFS Federation XML as a local path this issue occurs, example point to D:\ADFSFederation.xml

    1) You need to setup claims based point to the actual URL or

    2) Update DB table (Unsupported) to change from your local path to the URL path.
