We have two databases on the same server. A live environment and a test environment. Currently I swap between the two by using the "Select Server" option within NAV.
How can I customize 2 icons for NAV so that each one will default to the separate environments.

You can copy your ClientUserSettings file from "C:\ProgramData\Microsoft\Microsoft Dynamics NAV\100\ClientUserSettings.config", create two copies for test and live and change the connection settings in the files

Then you can create two desktop shortcuts, point them to the Nav Client.exe with the settings parameter like this:
"C:\Program Files (x86)\Microsoft Dynamics NAV\100\RoleTailored Client\Microsoft.Dynamics.Nav.Client.exe" -settings:"C:\YourFolder\ClientUserSettings_LIVE.config" "C:\Program Files (x86)\Microsoft Dynamics NAV\100\RoleTailored Client\Microsoft.Dynamics.Nav.Client.exe" -settings:"C:\YourFolder\ClientUserSettings_TEST.config"
