Dynamic 365 Portal Comment is not working .

Hello , I am new to dynamic 365 . I am working on a custom entity . Custom entity will be manupulated through dynamic 365 portal . I followed steps given in below link to add portal comment to my entity forms.

But when ever i try to add comment from portal , it is showing below error .
