msd crm 365 Custom ribbon buttons labels getting changed to CUSTOM

msd crm 365 on premise Custom ribbon buttons on one Custom Entity labels getting changed to CUSTOM & ALL ACTIONS ON BUTTON WORK AS EXPECTED !!!

- multiple custom entities created and custom buttons working well as expected - labels as well as commands & JS

- for the latest custom entity when i created custom buttons on form ribbon after publish it shows CUSTOM as seen below in screenshot

I Tried everything ->

checked entity name etc
Ribbon Workbench publish

Entity publish

solution publish

Cleared my browser cache
