How do I add fields to the Items table in GP 2015? I know in 2016 there is the new feature Extender but 2015 has nothing.

There is no way you would want to add a field within your inventory table without using a 3rd-party product like eOne Extender, as you might not be able to manage the fields properly from the front-end application..
Would you mind to explain more in details what you're trying to achieve ?
Extender isn't a thing of 2016, but was part of GP (under license) until GP2010, after which it was returned to eOne Solutions for continued dev & support. It is an ISV licensed product and therefore paying separate maintenance (and no longer part of the Microsoft distribution DVD).
