D365 Portals Form Not Loading

I created an Entity Form that should display the Account Web Form (Tab) on the Portal, however, the following error is showing up:

I checked the Entity Permissions and that the Entity Permissions is enabled and all seems to be fine. 

However, on the editor, the Tab is showing fine:

Does anyone have an idea please? Has anyone ever encountered this?

    I reconfigured the steps again from scratch, however, the same issue is popping up:

    Now, it is not appearing on the form editor too and instead it is showing the below error:

    Do you have any suggestions. please?

    Hi fran2105,

    You can check entity form metedata, mode and entity permission as following link said:

    Or you can try to restart to create new entity form as following steps:

    1.create a new entity form.

    2.create account entity permissions if you enable entity permission in the entity form.

    select web role that you want in current page, or you can create new web role.

    3.create web page.

    4.Sign into the portal and add account to the bar.

    5.click ‘Edit’ on the top right menu (see below screenshot) in the account page.

    The dialog box to customize the web page will pop up which will let you select the entity form that you created earlier at step 1.

    6.The portal will get refreshed after saving the changes on the dialog box and will render your entity form on it.

    After configuring, this is what the Dynamics 365 form section looks like in the Dynamics 365 Portal:
