How to Open a Canvas App from a Sitemap

Hi All, 

I wanted to open a Canvas App from a CRM Sitemap (Model Driven App). Means in Stemap there have a option called Open Canvas App. when i click on that then a canvas App will open. How to achieve the same. Any help will be much appreciated.
