I have a custom work order form created but when I access the actual work order it is displaying another form that I cannot find in the entity.
I don't even have option to switch forms like I do for bookings or other entities(Screenshot below)
Please advise!
You might want to check if your new form has been added to the app and also if the form has security roles assigned to it.

You don't see a form switcher because the user does not have more than one form available. And as Dian already pointed out, the one you see might actually be the fallback form that gets displayed if your security role doesn't give you access to any form.
Check the permission of the required form directly and also check which forms are activated in the definition of your app (likely the Field Service app).
