Hierarchy Quick View is completely grey

I followed this link to create a custom "hierarchy quick view form".

I created a new quick view form containing only the four field that I want to show.

The problem is that the quick view form is completely greyed out not showing anything... What could be the cause of this?

    Hierarchy Quick View is completely grey
    For me the forms were empty after I published only the customizations to account and model driven app, but started working (better) after I published all customizations.

    I also hid the column labels to make use of the whole square.

    Sorry, all I can say is that once I added the form to the App it all started working...that was it...

    Hi Fedorov, I have a question that if you didn't add the form to an app, then this form would not be displayed in the hierarchy form list. I met similar issue where all fields on a form in hierarchy are invisible, but the form is in the app. Could you tell me more details about your solution? Thanks!

    Ok, now it is solved and of course it all felt so obvious once the solution was presented..

    The new forms need to be added to the app..then it will work.

    Hello,

    I've cleared the cache (running Chrome incognito) with same result.

    I've downloaded Firefox and tried with same result.

    I finally tried with IE Edge and it was the same result..

    I have not yet tried the hierarchy for other entities because the customer only works with hierarchy on account level and support for all other hierarchys have been removed. I might be able to try at a later stage but anyways, there seems to be some problem/bug with the account hierarchy form..

    please clear the browser cache or change another browser.

    In addition, does Hierarchy Quick View of other entities have this issue?
