We needed a unique column which was not in the data set, so we used the "Add Column" using the "Index 1" type (Index starting a 1 and incrementing by 1).
The new column showed in preview correctly, and after hitting "Next", the query validated, but during the refresh after a few minutes, it just said "Failed".
No error code or diagnostics of any kind.
I found the resolution to the issue. It was not actually failing on the Index_1, it was failing for Non-unique columns in CI. Just interesting that it did not notice the non-unique columns until after I added the Index_1 to the column list. Once I resolved the non-unique column issue, the Index_1 works now.

Customer Insights does not accept any spaces or characters other than letters, numerals or underscores.
So try "Index_1".
