Account entity: Blank Ribbon in Chrome and Error when viewing form properties

I think these two issues are related. As long as I've been working at this company we haven't used the account entity at all, but I was tasked with getting it ready to use.

The ribbon bar of any account entity is completely blank when opened in Chrome or Firefox, but works in Internet Explorer. This only occurs for the Account Entity

In addition, when I open the "Form Properties" I get an error popup. There is one javascript file in the form library but it is empty when I open it. The view log button is also grayed out so the error is not very helpful.
