i wan' t call my custom lookup form in to form control on lookup event handler.
Can you share the sample code for that?

i am asking calling Custom lookup form in to on lookup event handler .
You can check for example Tables\LogisticsAddressCity\lookupCityId to understand how to launch a custom lookup form from x++ code. This method opens a form called LogisticsAddressCityLookup.
