Surface Related field on related entity (2 relationships away)

We have an Entity /Form Mapping/, which is related to /Campaign/, related to /Acknowledgement type/ and this has a look-up field to /Email Template/.
 
I'd like to be able to pull the /Email Template/ field on Ack Type on Form Mapping form.  I can bring through the name of the Ack Type using a Campaign QuickView form, but can't find a way to retrieve the Email template.
 
Any thoughts?  I think javascript may be needed.
 
I would rather not create the Ack type & Email template fields on the Form Mapping if possible.
