Customer Voice form disappears after load

Today I have noticed that my Customer Voice forms loads for a split second and then disappears leaving a blank screen.

They do this in all browsers and on mobile devices, and the same happens whether I share a link or email the form.

I can only get the form to appear in a browser in preview mode currently.

Is anyone else getting this? Is there any way to resolve this?

Looking at it in more detail, it appears to be forms which have a file upload question which are affected by this.

Having the same issue -- our Form has a file upload option and currently is not loading/only shows a blank screen. Haven't figured out why yet. :(
