You can create a custom entity and associate it to case. That should enable your scenario.

As Andrew mentioned below, you can create a custom entity called "Questions" and add your list of questions and associate it with the case entity that will work for your requirement.

I am quite confused with the requirement here.. is questions a 1:N for the case?
is it just fields within the case? is that part of creating the case properly?
can you elaborate a bit more the requirement
