Yes the actual development model will be supported also for D365BC On-Premise. You could choose if using the new extension model or the actual C/AL. Extensions are mandatory for D365BC SaaS.

You need to contact your local Microsoft headquarter and they'll give you all the materials for the addon submission.
In order to submit an addon, you need:
1) to have a valid contract with MS
2) Compile some documentations that MS will give you, especially an Excel sheet with objects in your addon (tables, pages, codeunits and so on)
After the approval, MS will assign you a valid range for the addon.
