Hello,
This is more of a general customisation question for Dynamics 365 sales Professional.
Is it possible to create a form for the salesman to fill out?
The idea is that when a salesman has called a client we want them to fill out some information about the call.
This will be a sort of survey for our internal clients if that makes sense.
I am sure i will need to create a custom entity, but i first want to know if this is possible and if so what would you recommend my approach be?
Any guidance is appreciated!

Yes you can create a Form for your sales man and then enable security role for your Form.
You need just to follow these steps :
1. navigate to make.powerapps.com and then navigate to your table
2. Create your FORM.
3. Select your Form, then clik On the 3 points then select form settings as shown in the figure below
4. Select Security roles, Select specific security role as shown in the figure below
Then you assign this to the role of your sales man.
It's appreciable that you create a specific security role for you sales man

There is no need to create a custom form or custom entity, You can create a Phone call activity against your lead in the timeline section and add the call’s information in the Description of Phone call activity form.
