Incorrect conversion between bin UoM and base UoM in warehouse management

We are using NAV 2018 CU 5

Scenario:

              - we have a bin in which there is an item that uses different unit of measure (UoM) (e.g CASE is the bin UoM, PC is the base UoM)

              -There is conversion between the two UoM (conversion is 12 pcs/case)

Issue: 

            1. There are times that when we do  movements from putaway bin to pick bin, incorrect "base qty" is posted in the warehouse entries. The base qty.' becomes 1 instead of                   12                 

            2. There are times that when we register picks, the qty picked (base) in the warehouse shipment  is incorrect. The qty picked (base) becomes 1 instead of 12. As such, we                    cant post the warehouse shipment due to this issue.

We tried to replicate and find out the cause of this issue. And sadly, couldn't find the cause. Sometimes it's okay and sometimes not. In fact daily, we have 30,000 warehouse entries and 200 entries are affected by this error. Seems like its random without any pattern why this error is happening. Please help!

    Bear in mind you have only three units of measure. Base, Purchase & Sale. The Purchase and Sale are for the Vendor/Customer facing documents. This way Purchase Orders and Sales Orders have appealing optics. But actually all transactions  are done on the Base UoM level. This is the reason your Picks , Putaways and such are in Base UoM.
