Unable to Qualify Leads - Gives a message - "Make sure all required fields are complete"

We have a leads qualification process that seems to be broken all of a sudden. Unable to make out what is causing the issue. All required information has been filled in and when trying to submit, gets the following message with no other errors. Unable to figure out what required fields it is complaining about. When reviewing the LeadsToOpportunity  Process, found a few steps referring to some fields that were removed from the form. But despite removing them from the process, the same error persists. Please help to narrow down what could be causing the issue.

    Hi Partner,

    If you have checked all the required fields on the Lead entity itself and there are no problems, you can check required fields on the related entities.

    By default, when you qualify a Lead you can create a number of records namely: Account, Contact and/or Opportunity.

    After you choose to Qualify a Lead Microsoft Dynamics will populate mapped data fields from the Lead to the newly created Account, Contact and Opportunity records.

    You need check required fields on these three entities(Account, Contact and/or Opportunity).

    And check that these required fields are mapped to the fields on the Lead, and if so, populate the corresponding fields on the Lead.

    Thank you for the guidance, explored this and found that there were 2 fields that we removed from the form that was part of the mapping. We removed them from mapping hoping that it will address the issue. but the situation still prevails. Interestingly a snapshot copy of production made in sandbox from when these changes were made seems to work OK, so really confused about what is happening, Will continue to explore with this information and see if we can figure this out.
