It uses SQL Azure for the database on production environments, so the whole database layer is abstracted, you don't have to worry about AlwaysOn and scaling out. Microsoft Operations should do this automatically for you if/when it's required.
Vilmos is correct about the sizing. You have to complete a questionnaire and specify numbers of master data and expected transactions per time unit. This needs to be done before the production environment will be deployed. Based on the answers, Microsoft will determine which hardware and setup is required. You cannot control it yourself. Based on telemetry Microsoft can determine if the sizing on Azure is correct.
An interesting question We were advised by Microsoft that that there no 'server' licences for DMO365 only user licences and therefore you can have as many AOS as you like however- how that gets set up in a production jnstance we can't touch is a moot point. If we are talking about a load balancing AOS then is it needed anymore with the cloud platform?
(A parallel question is what is the license for retail store servers i.e. there no ,longer appears to be a need to license those. Nothing in the price list.)
Based on your initial sizing you get the correct amount of instances deployed.

Default Production Instance
Each Dynamics 365 for Operations subscription offers one production instance per tenant. The production instance comes with disaster recovery and high availability.
That means it is already deployed like that for you if I am not mistaken. I do not have an AX 7 prod deployment to verify, but most likely it uses AlwaysOn or some kind of VM protection for that. I cannot find the exact details documented, maybe ask Microsoft directly through your Partner Account Manager.
For AX 2012 it was a different story, you had to option to go HA or not:
