Linking to CRM from our ERP system

We have an ERP and Project Management system that our team uses to manage proposals. We sync the proposals to CRM (read only) but often they would like to hop into CRM to add an activity to the proposal record. We would like to add a hyperlink from our ERP/PM system to the particular record in CRM so the notes/activities can be added. Although we have the proposal records in CRM, we do not sync any data back from CRM to our ERP/PM system, so we do not store the GUID anywhere outside of CRM. We do have a unique identifier on both systems that can be used to match them up. Any idea how this can be accomplished? Thanks!
