The requirement is to disable and enable form extension customizations based on a flag.

If flag yes - show standard form along with the customizations.

If flag No - show standard form alone.

If the flag is to be global for the environment, and could be extended to other functionality, consider using a config key.

Is it possible to enable and disable form extension customizations based on a flag ?

Yes as Martin suggested, you can't turn off complete extension but you can control all the customisation like let's say the code, or custom controls or custom fields what have been created for this customisation.

One way is to create a parameter and based on that enable / disable all customisation.

Second way is by configuration key but this requires maintainence mode to be enabled and disabled.

You can't turn off an extension, but you can write code to hide all controls (or other changes) added in the extension.
