Hi,
We are facing issue in CRM portal. Notes display properly but adding/Editing button of notes entity not display.
I have given all the permission as https://nishantrana.me/2017/02/27/configuring-note-for-entity-form-in-portal-in-dynamics-365/
We have configured same thing with demo CRM instance and it works well.

Could you try following
1. Clear Portal Cache If not already done so
2. Restart Portal from Portal Admin Centre, if not already done so
3. Turn "OFF" and "ON" Portal State from Portal Admin Centre

Hello,
We found solution for the issue.
It is due to roles. When we create portal we have standard two roles of Account & contact notes. We deleted this two roles and created own roles that is why attach button not seen in account form in portal.
