Customer Voice Survey Response activity not being created

We can send out a Customer voice survey and the Customer voice Survey invite activity is created. But when a customer responses to the Survey (fills in the survey) The Customer voice Survey invite activity is updated and Survey Invite Status = Responed. But what is not happening is a Customer Voice Survey Response activity being created.

So we can't see what the customer filled in the survey.

    Hi Sailesh, have you tested it, are you sure they are completing the survey? Reason I ask is we have had a lot of survey statuses show as started/opened when we know they haven't been
